Yes, my siblings and I usually gift each other on birthdays. Sometimes it’s something small but thoughtful, like a book or favorite snack, and other times we plan something together like a meal or a surprise.
We are four siblings. We don't give them gifts always but we always make sure to wish them personally, through phone calls, or social media posts.
We do this often and it's become a family ritual. My siblings and I share a very strong bond and we show appreciation for each other, especially during our birthdays or our children's birthdays or any other kind of family related event. We are always there for one another and cheering each other on all the time.

Me and my siblings I do not tend to gift to one another on birthdays and instead, we wait until Christmas and will gift then. This is something we have agreed to, and on birthdays, we end up sending birthday messages and possibly even giving them a card on their birthday.
I guess with the rising costs, it was something we all agreed with, so that if one couldn't afford to do something on a specific birthday, they would not feel guilty.

We all grew up fairly poor, so there really wasn't any gift-giving on any occasion between siblings (and hard for my mom to even get us the slightest of things on birthdays and Christmas).
So, we kind of kept up the tradition of just wishing a happy "X", even though we're all better off now. We know that means much more than a gift, so it works for us.

Not all the time but sometimes. I don't have any expectations on any of my siblings or parents to gift me on my birthday. It was only on my 20th birthday which I celebrated in a big I had more gifts from family and friends.
